This script does not use SMTP. It uses the mail function build in PHP

It looks like your hosts with godaddy is on the windows server. Ask godaddy to switch your host to Linux.
In an case, godaddy should make sure mail function is working for the PHP scripts.

If you still have problem, I have have it set up and hosted on my server (see the reame.txt file)

Thank you irhiggs for the correction. Correction has been updated in the release package.

This is simple a database setting. Look at the items in the VNSB_setting table to make sure they match with the actual filename. Remember to look at the table item and not the structure of the table.
